SUMMARY: Dr. Ben Mutiso from the Kijabe Mission Hospital outside of Nairobi Kenya, with its large Pediatric patient population describes how they approach removal of Posterior Fossa Tumors in Kenya. With more limited resources, he uses standard sub-occipital approaches we used 30 years ago with success, with no monitoring, or neuro-navigation, but with careful dissection to  achieve good results. What is challenging are lesions located in deep structures which are at the limits of the microscope, a situation faced by many around the world in HIC>LMIC.  Although there appears to be an intuitive benefit to new technology, what are the cost-benefits of these advances in limited resource settings, including the USA , where debt has replaced cost-effectiveness. The same questions can be asked for spine surgery. It is time to understand that although we can make cars with 1000 horse power; will we require it or use it for our needs? And there is no shame, if one does not have that unique car. Or for those special circumstances should there be a limited number of these cars in the country?
